Supervisor:                  Community Leadership and/or Health Director

 

Under the direction of the health director, the case manager/project coordinator will plan, organize, implement, direct, supervise, coordinate and evaluate activities related to the Jordan’s principle-Child first initiative project.

Roles and Responsibilities:

  • Ensure the well-being of clients into the program and promotes their development
  • Support to children with complex/special needs and their families including respite care, crisis intervention, behaviour management, and life skills training, whether on a one-to-one basis or in a group environment
  • Plans, develops, implements, and evaluates recreational, social and/or educational activities for children and their families
  • Strive to create a fun and enjoyable atmosphere to encourage client participation in programs
  • Ensure systems are in place support the delivery of the model of service
  • Taking the lead in securing effective development, delivery, and review of services
  • Ensuring workforce requirements, training and development needs, funding arrangements and outcome/performance measures
  • Developing a strategy that ensures all children are fulfilling their potential and that action must be taken to improve the lives of these children and their families who need help the most
  • Recognizing the deficits within service systems and the need to protect children through early interventions sharing of information, effective co-ordination of services and appropriate training of staff
  • Work in collaboration with the Prairie Mountain Health Authority, West Region Treaty Health Services and Children’s Disability Services to ensure clients needs are met while work is underway to create efficiencies, shared training, assessment methodology and tools
  • Provides education regarding Jordan’s Principle program
  • Emphasizes health promotion and illness and injury prevention
  • Incorporates knowledge of behavioural sciences with teaching and earning principles when carrying out educational activities and uses strategies appropriate to the relevant target group
  • Uses clinical skills to assess the client’s health status to enable joint planning, implementation, and evaluation of appropriate interventions
  • Uses appropriate health promotion, illness, and injury prevention techniques
  • Provides information for families
  • Promotes and supports patient safety
  • Monitor and evaluate all tasks of the child development workers, respite workers and the programs
  • Identify persons requiring programming
  • Make recommendations for changes and improvements to the program, as required
  • Schedule work assignments for child development workers, respite workers and administrative assistant
  • Coordinate appropriate care and equipment including other community resources, as required
  • Establish and maintain a current, accurate, confidential client reporting system
  • Encourage clients and families to be involved in care
  • Liaise with all family, medical and other resources, as required
  • Other duties deemed necessary/assigned

Qualifications

  • Nursing, Social work, Teaching/Child Development background preferred
  • Minimum 3 years of community - based experience preferred
  • Active registration with College of Registered Nurses of Manitoba/Social Workers Association(s) is required
  • Remuneration based on applicants experience and education
  • Knowledge of health services and programs both federal and provincial
  • Some supervisory and administrative experience an asset
  • Computer Skills (Microsoft Word/ Excel), for reporting requirements ie: e-SDRT
  • Knowledge of First Nations Community Health resources & programs
  • Knowledge of First Nations Culture
  • Willingness to obtain training
  • Excellent time management and facilitation skills
  • Effective verbal and listening communication skills
  • Strong leadership, critical thinking, decision making and problem solving skills
  • CPR/ Basic First Aid
  • Criminal Record/ Vulnerable Persons checks - (must have been done within the last 6 months)
  • Child Abuse Registry (must have been done within the last 12 months)
  • Valid Driver’s License and reliable vehicle.
